Big Sky’s first Community Health Workers (CHW) are here to support you as you navigate health resources and can assist in finding services that best fit your needs.
There is no cost to meet with a CHW, and they offer individualized research into:
Medical providers
Mental health resources and providers
Financial assistance opportunities
Social service programs
Addiction and recovery support
Community connections
If you think talking to a CHW would be helpful to you, please fill out the referral form. This form takes less than five minutes to complete and helps our CHW team know how to best support you. A CHW will follow up with you to schedule a time to meet within one to two business days.
*Note: Our CHWs are not licensed mental health clinicians. If you are having thoughts of suicide or feel you need immediate support, please call 988 to speak with a crisis counselor at the Help Center in Bozeman – they are available 24/7.
**Our CHW team is generally in the office Monday-Thursday. If you contact them outside of these days, they will be in touch as soon as they are able, usually within one to two business days.
