Hope is WIA and Big Sky’s first Community Health Worker (CHW). The CHW is a free service that can help you connect to resources to improve your wellbeing and help you reach your goals. The CHW assists individuals in setting goals, finding resources to meet their needs, connecting them with services, and reducing barriers to care. Please see the web below to learn more about how the CHW can assist you.

If you are interested in setting up a time to meet with the CHW, please fill out the referral form, so she knows how to best support you. The CHW will follow-up with you to schedule a time to meet.

 

The CHW is available Monday-Thursday from 8am-6pm. If you contact her outside these hours she will be in touch as soon as she is able (within 1-2 business days).

 

*WIA's CHW is not a mental health clinician. If you are looking to speak with a licensed provider immediately, please call           406-586-3333 to speak with a counselor from the Help Center in Bozeman - they are available 24/7.
